Financial Information

Payment for services is expected before being seen by the physician. For your convenience, we accept VISA, Mastercard and most bank debit cards. All insurance co-pays will be requested prior to your scheduled appointment.  We also offer CareCredit, a medical credit card as an alternate payment option.

Payment for Surgeries

All surgeries must be either pre-paid or acceptable arrangements made with our Surgery Coordinator. In all cases, any insurance coverage deductibles and co-pays must be paid in advance of surgery date. Insurance

Please bring your insurance card with you on the date of your scheduled appointment. A copy will be made for our files.

If a co-pay is required, you will be asked to pay that on the day of your visit. Any balance that remains after your insurance coverage pays will be your responsibility.

If a referral is needed from your primary care provider, please make efforts to do this prior to your visit.

Our physicians participate in many of the area managed care plans. (see list). This participation is subject to change, so it is always advisable to contact your insurance company or our office to verify our participation.

Medicare

All of our physicians participate in Part B Medicare. Any deductibles and co-pays (20% of allowed amount) will be your responsibility. We will also bill your Medicare supplemental insurance if needed.

At the current time, we are not participating providers of any Medicare Managed Care plans.

Liability/Accident Settlements

Due to delays in receiving payment for services when the claim is related to an accident or liability case, we will ask you to pay for your services and settle the matter with the other parties involved.
